Admissions Process

 

• Admissions Packet – Interested parents should request an admissions packet from our school by calling the school office or e-mailing the Admissions Coordinator (See contact information below).

• Admissions Application and Fee – In order for the application process to begin, a completed application and any applicable fees must be submitted to the school.  Upon submitting these items, parents can set up an appointment for testing through the admissions or counseling office.
• Transcript – The admissions process cannot be completed without an official school transcript, which includes current courses (if applicable), recent grades (if applicable), and all available scores from standardized tests and achievement tests.
• Vital Records – Birth Certificate, EED & Immunization Forms (the last two only apply to 3K, 4K, and all elementary).
• Questionnaire & Recommendation Forms – All applicants are required to have recommendation forms completed by their principal/counselor and teacher at the school they last attended.  It is our request that the principal/counselor and teacher complete the recommendation forms and return them directly to the admissions office as soon as possible.  (It is recommended to provide them with a stamped, addressed envelope.)  In addition, students in grades six and above are required to complete the Student Questionnaire.
• Letters of Recommendation – All applicants must request letters of recommendation from two non-family members.  It is the sole responsibility of the applicant and his/her parents to ensure that letters of recommendation are completed and mailed to Monroe Academy.
